Avance Counseling conducts DUI/Alcohol and Drug Evaluations (Uniform Reports) for individuals seeking a reinstatement of their driver’s privilege through the Illinois Secretary of State.
During the evaluation process one of our certified and qualified counselors meets with the client to conduct a thorough interview and collect information regarding the petitioner’s alcohol and drug history, review any Notices of Summary Suspensions/Revocations, traffic tickets, results of chemical testing and to review the petitioner’s driving record.
In order to assist you with this process you will need the following:
- Please bring with you a current driver’s abstract. This driving abstract may be obtained at any Illinois driver’s license facility. The abstract document must state for court purposes.
- Statutory Summary Suspension(s) and/or Law Enforcement Sworn Report. This is a report that you received at the time of your arrest which shows your BAC (Blood Alcohol Concentration) or refusal.
- All and/or Available Treatment Records. Please bring a copy of your treatment discharge documents regarding your DUI treatment completion.
- If you have been previously denied driving relief from the Illinois Secretary of State we need a copy of the denial letter.
The evaluator will conduct a brief interview (in person or on the phone) with a family member or friend to verify the information you provided and to answer any additional questions or concerns.
We will also conduct a standardized test “Adult Substance Use and Driving Survey test” (ASUDS-RI test) to ensure proper classification of the DUI level of care.
NOTE: If this is the first time that you are required to complete a DUI evaluation and you were ordered by the court to complete a DUI evaluation, you must follow the following instructions since each courthouse has its own rules and guidelines about the DUI evaluation process.
If you were arrested in Cook County, you must contact the Alcohol and Drug Evaluation Services (ADES), which is operated by Central States Institute on Addiction (CSI).
