Substance Abuse Program/Substance Abuse Evaluations
Enrollment:
To enroll in the Substance Abuse Program, you must complete the Substance Abuse Evaluation:
1. Intake paperwork:
Please follow the link to complete the initial intake paperwork. Please use as much detail as possible and avoid leaving any questions unanswered.
2. Schedule a Follow-Up Appointment:
After completing the Intake paperwork, you will need to schedule a time to follow-up with a counselor about any additional program information.

After you have completed the forms, you must call Avance to schedule a follow-up appointment.
After your Follow-Up Appointment:
Evaluations:
After completing your evaluation, the counseling team will need time to determine any results or recommendations. We will contact you and your caseworker/probation officer/parole officer when your results are prepared.
Classes:
If you are required to complete classes, we will begin sending you the class information upon completion of your intake and follow-up appointment.

Tuesdays 11:00am and 6:00pm
Wednesdays 11:00am and 5:00pm
Thursdays 11:00am and 6:00pm
Saturdays at 1:30pm
Program Fees:
There are fees associated with all services. At Avance, we work on a sliding scale based on your income level and current financial situation. We will discuss fees with you during your follow-up appointment.
Upon Completion of all forms and follow-up appointments, we will begin sending you the links to join classes via text message.
